How to register
We have an open list and welcome the registration of patients living in or moving to the local area. Our Practice covers the areas around Ponteland, Darras Hall, Stamfordham and Wylam. The Practice boundary includes the following areas :-
- In the North - Cambo, Scots Gap, Angerton and Tranwell
- In the East - Stannington, Blagdon, Dinnington and Kingston Park
- In the South - Ovingham, Wylam, Heddon and North Walbottle
- In the West - Kirkwhelpington, Kirkharle, Hallington and Great Whittington
You can register by bringing along your NHS medical card or completing a form available from reception. Alternatively you can request a registration form by sending an email to admin@gp-a84011.nhs.uk - please include your postal address so we can verify that you are within our practice boundaries. We encourage prospective new patients to register as soon as you move into the area rather than waiting until you need to see a doctor as records from your previous GP may take some time to be transferred.
We recommend that new patients undertake a health check with a practice nurse.
Appointments
We run all our surgeries by using an appointment only system. Appointments can be booked by telephoning or visiting any of our surgeries in Ponteland, Stamfordham or Wylam.
We try to make sure you see the doctor of your choice but if they are not available we may ask you to see another doctor.
We provide a limited number of same day appointments for patients with urgent medical problems. We do not provide a walk in service for patients but will try to see all those patients who cannot wait, for medical reasons, for the next routine appointment.
If you decide that your appointment is no longer needed, please ring up and cancel so that someone else can have the appointment instead.
If more than one family member needs to see the doctor, please make a separate appointment for each person.
If you are too ill to come to the surgery and you require a visit at home, please call the surgery before 10am. Home visits take up much more of a doctor’s time than a consultation in the surgery, so if you are mobile please come into the surgery to be seen.

Prescriptions
If the doctor agrees you may obtain a repeat prescription for your medication. Please telephone or leave your request at your usual surgery. Your prescription will be ready for collection 48 hours later. If you prefer, we can post it back to you (you will need to provide a stamped, addressed envelope).
We offer a dispensing service for those patients who live more than one mile from their nearest chemist. Please ask if you are eligible for this service. We dispense during normal surgery hours. If you are eligible to use the dispensing service we ask that you order your prescription from the surgery you intend to collect the prescription from (or alternatively use our internet ordering service).
We are keen to ensure that patients with ongoing medical problems are monitored regularly. If the date for your next review with a doctor or nurse has passed you will be asked to make an appointment.
Sick certificates
You only need a doctor’s note if you are unable to work and are ill for longer than seven calendar days. Your employer will provide you with an SC2 self-certificate form for shorter periods of illness.
If an employer insists on a doctor's note for less and one week's absense then we will charge a fee to issue a private certificate.
If you have been in hospital, a sick note can be issued by your hospital doctor or consultant without the need to see a GP.
Test results
To ensure confidentiality, we only release results to the patient, unless alternative arrangements have been agreed in writing.
Blood & urine tests can take up to 10 working days before the results are available.
Non-NHS services
The NHS does not pay for some of the services we provide. These include private sick notes, medical examinations & reports, insurance forms, holiday cancellation forms, medical reports, fitness to travel certificates, private prescriptions, passport signing and some vaccination services. Our fees for these services are displayed in the surgery.
